Authorization and Release

*PARENT/GUARDIAN SIGNATURE REQUIRED FOR SUBJECTS UNDER THE AGE OF 18*

This agreement is between Jill Nissen/The Proof Photography ("Photographer") and the above named subjects with respect to the images, photographs and/or videos created at this event ("Images").

I hereby give the photographer permission to take video and still images of me and in which I may appear, and to use these images and their derivatives in print and online display, social media sharing, advertising, publication, and all other lawful purposes without restriction. I understand that the photographer is the sole owner and copyright holder of these images, and I acknowledge that I will not receive any monetary compensation for the usage of my images. I hereby release the photographer from all forms of claims and liability relating to the taking and usage of these images.

By signing below, I state that I have read I have read this authorization and release in full, understanding its contents, and agree to its terms. This Agreement shall be binding upon me and my heirs, legal representatives, and assigns.
